If you would like t o become a new patient, please download and fill out the consent form and return to us. This form can be emailed or faxed back to us  info@tranquilityhousecalls.com or Fax: 248-742-9121.

 Tranquility Housecall Services accepts Medicare, Medicaid, and most major commercial insurance plans. Private-pay concierge options are also available for enhanced physician accessibility and urgent care support. 

Once received, we will contact you as soon as possible to ensure your intake is expedited.
